What is the best way to allow showing, hiding, swapping items in hand (sword, key etc.)? I thought I would make for example walk animation with sword, hide the sword and then in runtime show it when necessary.
I see than I can swap attachements, what about empty hand though?

Yep. Attach what you need in the editor, show the appropriate images in your game. There is no difference between a hand image and a sword image.

I was thinking about what to do when the hand is free - so no attachment should be shown. But probably I can hide the whole slot then or show transparent PNG. I'll test it next week.

Mmm at the editor I think it is possible to set the visibility of a skin attachment or slot, so I guess it would be possible at runtime. Haven't tried tho.

You can set the attachment for a slot to null at runtime to have no attachment.
